大黑天本是婆罗门教湿婆的变身，后为佛教吸收而成为佛教的护法神，特别是在密宗中，大黑天是重要的护法神，是专治疾病的医神。此像头戴五骷髅宝冠，身挂璎珞，披帛绕肩飞扬。主臂右手持钺刀，左手本应持嘎巴拉碗，现已佚失。其余手臂各持法器，结法印。其身姿挺立，神韵洒脱，超凡脱俗，令人叹为观止。
