提要：是造象记立于北魏永平元年（508）年，记述比丘运生、车骑将军张显、王府参军慕容晖等廿八人造象祈福，普济众人之事。是拓面板墨笔题签“魏慕容晖等造象”，首页签条书：“慕容晖像记出土即为日本人购去”。娄东周悫旧藏，此造像未见各家著录。
